Who needs OET for ECFMG?

International medical graduates on the 2027 Pathways need OET Medicine.

The OET Medicine requirement applies to international medical graduates applying through the ECFMG Certification Pathways for the 2027 Match. The test checks the clinical communication skills you will use with patients and colleagues in US residency, so ECFMG treats it as part of the communication requirement. Use the USA pathway guide for the wider route from ECFMG Certification to residency and state licensure.

Only OET Medicine counts for this purpose. The Nursing, Pharmacy and other profession versions are not accepted for ECFMG.

What OET score does ECFMG require?

Three sub-tests at 350 and Writing at 300, all from one sitting.

Sub-testMinimum scoreGrade
Listening350B
Reading350B
Writing300C+
Speaking350B
  • One test administration: all four scores must come from the same sitting. If you miss the minimum in any sub-test, you retake all four.
  • Date: the passing result must be from a test taken on or after 1 January 2025.
  • Retakes: ECFMG sets no limit on the number of attempts.

Enter your own scores in the OET score calculator to see whether they meet the ECFMG minimums.

Which OET test mode can you use?

Paper and computer at a venue are the default; OET@Home is the exception.

  • OET on paper: all four sub-tests on one day at a test centre. Paper bookings close 24 days before the test date.
  • OET on computer: Listening, Reading and Writing at a venue, with Speaking usually taken online on a different day. Bookings close 7 days before the test, and results can arrive in as little as 48 hours, with 95% released within 6 calendar days.
  • OET@Home: accepted by ECFMG only if there is no physical test venue in your country. You must sit it in the country you booked and show proof of presence at check-in.

OET lists the full test at AU$587, or US$455 for the computer and at-home tests in the USA. See OET test dates and fees for booking details.

How do OET scores reach ECFMG?

Your MyIntealth ID links your OET result to your ECFMG record.

From 14 May 2026, OET sends Medicine results to ECFMG using your MyIntealth ID. If your results are released on or after that date, make sure OET has your MyIntealth ID, not your USMLE ID.

  • If your MyIntealth ID or date of birth does not match, your results are not released to ECFMG.
  • Results released before 14 May 2026 under a USMLE ID are matched within 3 business days.
  • Allow 3 business days for a passing result to update in your Pathways application.

What are the deadlines for the 2027 Match?

Work back from 31 January 2027 and leave room for one retake.

ECFMG advises taking OET Medicine on or before the last scheduled test date in December 2026. Results that arrive later are still processed but may be too late for the 2027 Match. ECFMG says results are typically released about 10 business days after the test. Computer dates vary by country, so check the official OET date finder for sessions near you.

Step-by-step checklist

Follow this order to avoid a result that arrives too late or never reaches ECFMG.

01
Get your MyIntealth IDCreate or confirm your MyIntealth account and check your name and date of birth.
02
Choose the test modePaper or computer at a venue; OET@Home only if your country has no venue.
03
Book earlyPaper closes 24 days before the test, computer 7 days before. Make sure OET has your MyIntealth ID.
04
Prepare Medicine Writing and SpeakingThese two sub-tests decide most retakes for doctors.
05
Sit all four sub-testsOne administration; all four scores must meet the minimums together.
06
Track the resultCheck that your passing scores appear in your Pathways application.

Which mistakes delay ECFMG results?

Most problems are administrative, not linguistic, and each one can cost you the 2027 Match.

  • Using your USMLE ID: results released from 14 May 2026 need your MyIntealth ID; the wrong ID means the scores do not reach ECFMG.
  • A mismatched date of birth: if your details do not match, OET cannot release your results to ECFMG.
  • Booking OET@Home with a venue nearby: ECFMG accepts OET@Home only when your country has no test venue.
  • Counting on two sittings: ECFMG needs all four minimums in one test administration; a strong Writing score from an earlier sitting cannot be carried over.
  • Using an old result: the passing test must be taken on or after 1 January 2025.
  • Leaving no time for a retake: paper bookings close 24 days before the test and results can take around two weeks, so the last December dates leave no room to try again before 31 January 2027.

How should doctors prepare for OET Medicine?

Listening and Reading are the same for every profession; Writing and Speaking are where doctors usually need targeted work.

Writing: referral letters

Medicine Writing is usually a letter to a specialist or GP who will take over care. Select only the case notes the reader needs, state the purpose early and make the request clear. Practise with the OET Medicine writing sample.

Speaking: patient conversations

Role-plays include explaining a diagnosis, discussing treatment options and responding to a worried patient. Structure, empathy and checking understanding are marked alongside language.

A retake means all four sub-tests again, so check Writing with OET writing correction and follow the OET course for doctors.

FAQ

What OET score do I need for ECFMG?

For the 2027 Pathways, ECFMG asks for OET Medicine with at least 350 in Listening, Reading and Speaking and 300 in Writing, all in one test administration taken on or after 1 January 2025.

Can I use OET@Home for ECFMG?

Only if there is no physical OET test venue in your country. Otherwise you need to take OET Medicine on paper or on computer at a venue.

What is the MyIntealth ID used for?

If your OET Medicine results are released on or after 14 May 2026, you must give OET your MyIntealth ID, not your USMLE ID, so your scores reach ECFMG. If the ID or date of birth does not match, the results are not released to ECFMG.

When should I take OET for the 2027 Match?

ECFMG advises taking OET Medicine on or before the last scheduled test date in December 2026. ECFMG must receive your scores directly from OET by 31 January 2027.

What happens if I miss the score in one sub-test?

You must retake all four sub-tests in a new test administration. ECFMG does not limit the number of retakes, and OET does not offer single sub-test resits in the USA.

Aiming for the 2027 Match?

There is little room for a retake before 31 January 2027. Check your level with the free placement test, then prepare Writing and Speaking with the OET course for doctors.
