The USA is state based, not one national licence
National exams exist, but the actual licence is issued by a state board. That means the correct English test, credential report and application route can change by profession and state.

Doctors - ECFMG / Intealth pathway
For international medical graduates aiming for ECFMG Certification, ERAS, the NRMP Match, residency training and later state medical licensure. OET Medicine is part of the communication skills requirement for Pathways applicants. See the OET course for doctors for ECFMG-focused preparation.
English-test planning: Official ECFMG 2027 guidance requires OET Medicine for Pathways applicants, with 350 in Listening, Reading and Speaking and 300 in Writing in one test administration. Always follow the current ECFMG rules.

Nurses - State BON / NCLEX pathway
For internationally educated nurses applying state by state. NCSBN owns the NCLEX-RN, but each Board of Nursing decides application requirements, credential evaluation, English testing, fingerprinting, SSN policy and authorization to test.
English-test planning: Do not assume OET Nursing will count for US nursing. TOEFL iBT or IELTS Academic is often the English route for US nurses, depending on the state board; CGFNS VisaScreen also accepts OET.

OET / English preparation focus
Writing
If you also target the UK, OET Nursing supports NMC preparation, but it may not support your US BON route.
Speaking
NCLEX communication preparation should focus on patient safety, prioritisation and clinical judgement.
Reading
Read state board instructions carefully, especially concurrency and report-type language.
Listening
Prepare for US accents and workplace communication, but choose the English test your target authority accepts.
Pharmacists - NABP / FPGEC pathway
For foreign pharmacy graduates seeking FPGEC Certification before state board eligibility, NAPLEX, MPJE or a state law equivalent, internship hours and final licensure.
If your route also needs OET (for example for UK, Irish or Australian registration), see the OET course for pharmacists.
English-test planning: NABP/FPGEC requires TOEFL iBT as part of FPGEC Certification. Current NABP guidance says all four sections must be completed in one testing session, MyBest scores are not accepted, and remote-proctored/at-home TOEFL is not accepted.

Create NABP e-ProfileUse the correct NABP account and follow the current Candidate Application Bulletin.
Prepare TOEFL iBT correctlyUse the current NABP score scale and test-location rules before submitting the FPGEC application.
Submit ECE and documentsArrange ECE evaluation, license evidence, identity documents, translations and score reports.
Pass FPGEE then state examsComplete FPGEC Certification, then NAPLEX, MPJE/state law exam and internship hours as required.

- Booking OET Pharmacy for FPGEC.

OET / English preparation focus
Writing
OET Pharmacy can help UK or Australia planning, but it does not replace FPGEC TOEFL rules.
Speaking
Pharmacists need English preparation aligned with TOEFL iBT if the sole target is the US.
Reading
Read NABP document-sealing and ECE instructions carefully to avoid delays.
Listening
Plan FPGEE timing because exam windows and certification timelines are strict.
Physical therapists - FSBPT / State PT pathway
For foreign-educated physical therapists applying through state boards. Each state decides evaluation agency, English testing, supervised practice and jurisprudence rules, while the NPTE is administered nationally through FSBPT.

English-test planning: TOEFL iBT is the usual English-test route for US physical therapists. Do not book OET Physiotherapy for the US unless your exact state board explicitly confirms acceptance; OET Physiotherapy is primarily useful for UK HCPC and selected non-US bodies.

Do USA doctors need OET Medicine?
Yes, current ECFMG Pathways guidance requires OET Medicine for Pathways applicants and states there are no exceptions for native language, medical school language or citizenship.
What OET score does ECFMG currently require?
Current 2027 ECFMG guidance lists 350 in Listening, Reading and Speaking and 300 in Writing in one test administration. Candidates must verify the current ECFMG cycle before booking.
Can nurses use OET Nursing for the USA?
Not automatically. US nursing is state-board dependent, so check your Board of Nursing. CGFNS VisaScreen does accept OET: for tests taken on or after 12 May 2026, registered nurses need Speaking 360, Reading 340, Writing 350 and Listening 320, and at-home tests are not accepted.
Is US licensure the same as a visa?
No. Licensure, certification, credentialing and visas are separate. Candidates should verify official licensing rules and seek licensed immigration advice for visa questions.
